BioZyme Inc., maker of Sure Champ by Vita Ferm, supports America’s youth and is committed to the success of our future leaders in agriculture. Through a licensing agreement with the National FFA Organization, BioZyme Inc. will kick off the Sure Champ FFA Chapter Fundraiser at the 2011 National FFA Convention, Oct. 19-22. This program is based on the collection of special Sure Champ $1 FFA Rewards Tokens that can be redeemed only by official FFA chapters to benefit their local chapter and programs.

How the Program Works:
- Beginning November 15, start watching for Sure Champ $1 FFA Rewards Tokens in identified bagged
and bucketed products. For every Sure Champ $1 FFA Rewards Token collected by FFA members and
postmarked to Sure Champ on or before July 30, 2012, Sure Champ will donate $1 to the FFA chapter
redeeming the tokens with the Sure Champ Rewards Submission Form. To find your closest Sure Champ
dealers visit www.surechamp.com or if you don’t have a dealer near you call 1-800-821-3070.
- There is no limit to the number of legitimate Sure Champ $1 FFA Reward Tokens FFA chapters can
collect from either their own Sure Champ purchases or from Sure Champ purchases made by family,
friends, FFA supporters or livestock breeders. Customers must complete the dealer and other information
on the back of the token and submit the token to the local FFA chapter advisor.
- FFA members will accumulate these tokens, and turn them into their FFA advisor. The FFA
advisor will then submit the tokens and submission form to the BioZyme Inc. corporate office. In turn
BioZyme Inc. will donate $1 per authenticated token back to the listed local chapter following verification
of the tokens and the chapter’s good standing. (A completed Sure Champ Rewards Program Submission
Form, available at www.surechamp.com, must be included with all submissions).
- All claims for redemption must be made at the chapter level. FFA chapters may redeem the tokens
weekly, monthly or at the end of the promotion as long as tokens are accompanied by a completed Sure
Champ Rewards Submission Form and are postmarked on or before July 30, 2012 no exceptions. In turn,
BioZyme Inc. will donate one dollar ($1.00) per token redeemed back to the FFA chapter who sent in the
tokens and the Sure Champ Rewards Submission Form. All donations will be made to official FFA chapters
no later than Sept. 30, 2012.
- Additionally, Sure Champ will randomly insert Sure Champ Instant Winner Tokens in a limited number
of Sure Champ bagged and bucketed products. The Sure Champ Instant Winner Tokens are redeemable
for a variety of items (including Sullivan Supply gift cards and Sure Champ apparel). FFA chapters or
members finding the random Instant Winners Tokens will need to call the BioZyme Inc., corporate office
to claim their prize. Instant winner tokens do not qualify for the Sure Champ FFA Rewards Program.
- Sure Champ reserves the right to disqualify any tokens that have been replicated, defaced, mutilated, forged or in any way altered. Sure Champ will be sole judge regarding the disqualification of tokens due to replication, defacing, mutilation, forgery or alteration of Sure Champ $1 FFA Rewards tokens. All tokens sent in for submission must be accompanied by the Sure Champ Rewards Submission Form, no exceptions. Any chapter advisor promoting the sale of or knowingly submitting fraudulent tokens will be subject to expulsion from the Sure Champ FFA Rewards Program.
